Auto Insurance Form

This form is required for all field trip drivers.  Please include a photocopy of your license and auto insurnce card when returning the form.

Note: Any drivers not meeting the requirements set forth in the Auto Insurance form may not drive any private vehicle in connection with transporting Holy Spirit School Students for any school-sponsored activities.
