Enrollement Application Form

HOW TO APPLY FOR ADMISSION

1.   Complete the enclosed APPLICATION form. Please attach a family photograph.

2.   Kindergarten applications will be accepted December 1-January 28th. Available spaces will be determined and all kindergarten applicants will be notified of acceptance by March. Applications for grades1-8 are accepted throughout the school year. For fall admission, applications for these grades should also be submitted by February 15th.

3.   If you are an applicant for a Kindergarten position, you’ll need to attend the Kindergarten Information Night on Tuesday, January 10, 2012 @ 6:30pm in the Holy  Spirit School Father Brady Room (room next to gym).

4.   Please bring the following items with you to Kindergarten Information Night:
• The APPLICATION form (if it has not been submitted)
• Birth certificate (copy)
• Baptismal certificate (copy)
• Immunization record (up to date copy)
• Picture of your family

5.   For those who have been notified of acceptance, registration packets will be available for download online at www.hs-ps.com (under forms) on April 1, 2012 or in the school office.

6.   Registration Day will be held on Tuesday, April 24, 2012. You may attend anytime between  3:00pm and 6:00pm
 
7.   Applicants for grades 1-8 will be placed on our Waiting List. As openings occur, you will be notified and your registration will be processed.
